The Ponemon Institute released the 2016 Cost of Insider Threats study this fall. Over 68 percent of the incidents reported were due to insider negligence. 

The IBM i world is not immune to this issue: new administrators or operators are hired and don’t receive the proper training, or an upgrade occurs that changes settings. And because no one has sufficient time to get things done anymore, shortcuts are taken or tasks are forgotten or re-prioritized.
